Access to medical records

The practice is registered and complies with the Data Protection Act 2018 (DPA 2018). Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please find information on the patient information page about how to request this and the necessary form that needs to be completed.

Access to Records Online (Patient Access)

You can go online and see information in your GP record, including medications, allergies, test results, immunisations and illnesses. You can also order repeat prescriptions online. Appointment booking is not available online due to our appointment booking process.

You can look at your GP records on a computer, a tablet or a smartphone using a website or app. If you would like to start using online services see the “Getting started with GP online services” guides for more information. The easiest way to register for this service is by downloading and using the NHS app

Anything added to your medical record from the 1st November 2022 will be visible to you automatically. If you wish to have access to your medical records prior to this date you will need to apply to the Practice for this.

  • Any information in your online record is important and private, so you should keep it safe and secure. Never share your
    medical information with others on any social media.
  • Make sure you log out when you finish looking at your information, so that no one else can see it or change your password
    without you knowing. Make sure your password is not remembered by the website either.
  • If you lose or forget your login details, go to the website which you normally use and click the ‘forgotten details’ button, then
    follow the instructions. If this does not work, please contact us.
  • If you think that someone else may have unauthorised access to your online account, firstly try to change your password,
    then contact us to audit your online accounts and reset your online account if needed.
  • No one should force you to share your username, password, or any information in your online record. You have the right to
    say no. If someone asks to see your records and you do not want them to, please let us know as soon as you can.
  • If you change registered GP Practice, you will need to register again for online services at your new GP Practice.
  • Your records are written to help medical staff look after you and so in some cases, you may not understand everything you
    see. If you find anything difficult to understand, go to the NHS website abbreviations guide. You can also go to the NHS Choices
    website. NHS Choices is the NHS website for patients where you can look for information on illnesses, improving health
    and find NHS services in your local area. Another website frequently used to search for information on test results is Lab Test Online UK.
  • Information in your GP record might need correcting. If you find something you think is not correct, you should contact us.
    The staff will be able to answer your questions and set things right when needed. Please bear in mind that you cannot
    change the record yourself.
  • There may be information in your GP records that you did not know was there or that you had forgotten about, such as an
    illness or an upsetting incident. If you see anything you did not know about that worries you, please discuss this with us.
  • If you see someone else’s information in your record, please log out immediately and let us know as soon as possible.

If you are not able to use the NHS app to sign up for online services you can do this by coming into the surgery. You will need to complete:

  • The relevant consent form; Individual Consent Form Proxy Access Consent Form
  • Bring in 2 forms of ID, one photographic and one with your address on;
  • If you are a a parent requesting access for a child between the age of 11-16 both you and your child will need to attend the surgery

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access:

  • fill in our proxy access consent form, and bring to reception. You will need to bring in 2 forms of ID, one photographic and one with your address on.
  • collect a proxy access form from reception
  • If you are a parent requesting access for a child between the age of 11-16 both you and your child will need to attend the surgery

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
